Please have your paperwork completed by the time of your first appointment and arrive 10 minutes early with your current insurance card and ID. This first appointment will be the longest to allow time to go over history, general medical information, conduct an exam, provide treatment and create a treatment plan. See below for current Covid19 protocols and be prepared to answer screening questions at the time of your appointment.
